Paciente masculino, 65 anos, em avaliação pré-anestésica. Ao ECG, apresenta ritmo sinusal com sobrecarga ventricular esquerda (por critérios de Romhilt-Estes), representada por:  área da fase negativa de P de pelo menos 0,04 mm/s, ou igual ou superior a 1 mm2 (índice de Moris); aumento de amplitude do QRS (20 mm no plano frontal ou 30 mm no plano horizontal); alterações secundárias da repolarização ventricular, com infradesnivelamento do segmento ST e onda T negativa (Padrão strain). Observa-se, também, extrassístoles ventriculares e intervalo QT prolongado.

On ECG, tracing shows sinus rhythm with signs of left ventricular enlargement (by the Romhilt-Estes criteria), characterized by: area of the negative phase of P wave of at least 0.04 mm/s or equal to or greater than 1 mm² (Moris index); increased QRS amplitude (20 mm in the frontal plane or 30 mm in the horizontal plane); secondary ventricular repolarization abnormalities with ST segment depression and negative T waves (strain pattern). Ventricular extrasystoles and a prolonged QT interval are also observed.
