Paciente masculino, 49 anos, com queixa de dor torácica e palpitações. Tem diagnóstico de hipertensão arterial sistêmica. Em uso de Furosemida, Metoprolol, Isossorbida e Hidralazina.  Ao ECG, apresenta ritmo regular sinusal, com sobrecarga biventricular, representada por eixo elétrico de QRS no plano frontal desviado para a direita, associado a critérios de voltagem para SVE,SVE; ECG típicosugestivo de SVD, associado a um ou mais dos seguintes elementos: R de voltagem aumentada em V5 e V6.V6 e S de V1 + R de V5 e V6 com critério positivo de Sokolow. 

A 49-year-old male patient is experiencing chest pain and palpitations. He has been diagnosed with systemic arterial hypertension and is currently taking Furosemide, Metoprolol, Isosorbide, and Hydralazine on a daily basis.hypertension. The ECG shows a regular sinus rhythm with biventricular enlargement, indicated by a rightward deviation of the QRS electrical axis in the frontal plane. There are also voltage criteria consistent with left ventricular overloadhypertrophy (LVO)LVH) and electrocardiographic findings typicalsuggestive of storeright voltageventricular overloadhypertrophy (SVD)RVH). Notable findings include increased voltage R waves in leads V5 and V6, as well as the S wave in V1 along with the R waves in V5 and V6 meeting the positive Sokolow criterion.
