Paciente feminina, 45 anos, com queixa de palpitações. História familiar positiva para doença coronariana. Ao ECG, apresenta ritmo sinusal regular com Sobrecarga Atrial Esquerda, com onda P com duração de 144 ms, considerada aumentada, com duplo pico separados por mais que 0,04 s em DII e com área da porção negativa da onda P em V1 maior ou igual a 40 mm/s. Observa-se, também, alterações inespecíficas da repolarização ventricular. 

A 45-year-old female patient is experiencing palpitations and has a positive family history of coronary disease. The ECG shows a regular sinus rhythm with signs of left atrial overload. The P wave duration is 144 ms, which is considered prolonged, and it exhibits a double peak that is separated by more than 0.04 seconds in lead II. Additionally, the negative portion of the P wave in lead V1 has an area greater than or equal to 40 mm². There are also nonspecific changes noted in ventricular repolarization.

Paciente masculino, 65 anos, com queixa de dor torácica. Ao ECG, apresenta ritmo sinusal com Sobrecarga Atrial Es querda, representada por onda P tem duração de 152 ms, considerada aumentada , com duplo pico separados por mais que 0,04 s em DII e com área da porção negativa da onda P em V1 maior ou igual a 40 mm/s. Observa-se também, extrassistolia ventricular monomórfica e isolada.

The ECG shows a sinus rhythm along with left atrial overload, indicated by a P wave duration of 152 ms, which is considered prolonged. The P wave exhibits a double peak, with the peaks separated by more than 0.04 seconds in lead II, and the negative portion of the P wave in lead V1 has a larger area, equal to or exceeding 40 mm/s. Additionally, monomorphic and isolated ventricular extrasystoles are observed.
