Paciente masculino, 78 anos, com queixa de edema em membros inferiores, Nega comorbidades ou uso de medicamentos. Ao ECG, apresenta ritmo sinusal regular com Sobrecarga Atrial Direita, representada por ondas P apiculadas em D2, com voltagem superior a 2,5 mm em DII e porção positiva de V1 > 1,5 mm. É possível observar, também, a presença de sobrecarga ventricular direita, desvio extremo do eixo QRS e alterações secundárias da repolarização ventricular em derivações precordiais direitas, com infradesnivelamento do segmento ST e onda T negativa. 

A 78-year-old male patient is experiencing edema in his lower limbs. He denies having any comorbidities or taking any medications. The ECG shows a regular sinus rhythm along with signs of right atrial overload, indicated by peaked P waves in lead II, with a voltage greater than 2.5 mm in lead I and a positive portion of lead V1 measuring over 1.5 mm. Additionally, there are indications of right ventricular overload, an extreme deviation of the QRS axis, and secondary changes in ventricular repolarization observed in the right precordial leads, including depression of the ST segment and negative T waves.
