Paciente masculino, 73 anos, com queixa de cefaleia.Tem diagnóstico de hipertensão arterial. Em uso de Losartana. Ao ECG, observamos a presença de três morfologias distintas da onda P no DII longo, compatível com ritmo atrial multifocal. Observa-se, também, prolongamento do intervalo QT. 

A 73-year-old male patient presents with a complaint of a headache. He has been diagnosed with high blood pressure and takes Losartan daily. The ECG shows three distinct morphologies of the P wave in lead II, which is consistent with a multifocal atrial rhythm. Additionally, a prolongation of the QT interval was noted.

Female patient, 70 years old, elective examination. On the ECG, we observed the presence of three distinct morphologies of the P wave in long DII, which is compatible with a multifocal atrial rhythm.
