Paciente masculino, 54 anos, em avaliação pré-anestésica. Ao ECG, apresenta ritmo sinusal regular, com eixo de QRS entre -30° e -90°; QRS predominantemente positivo em DI e negativo em aVF.

Paciente feminina, 67 anos, com queixa de dor torácica. Tem diagnósticos de hipertensão  e obesidade. Ao ECG, apresenta ritmo sinusal regular, com complexos QRS predominantemente positivos em DI e negativo em aVF, correspondendo a um eixo de QRS entre -30° e -90°.
