Paciente masculino, 31 anos,  exame realizado para acompanhamento clínico.  Apresenta HAS e história familiar positiva de doença coronariana. Ao ECG, apresenta QRS alargado, com padrão rsR´ em V1 e ondas S empastadas em V5 e V6. Eixo mais negativo que -45º, desviado à esquerda, d com padrão qR em aVL, compatíveis com Bloqueio de Ramo Direito e Bloqueio Divisional Anterossuperior do ramo esquerdo. Observa-se, também, sobrecarga atrial esquerda.

A 31-year-old male patient came in for a routine follow-up. His medical history includes hypertension and a family history of coronary artery disease. The ECG showed a right bundle branch block (RBBB) and left anterior fascicular block (LAFB). This was characterized by a widened QRS complex, an rsR' pattern in lead V1, and slurred S waves in leads V5 and V6. The QRS axis was deviated to the left, exceeding -45 degrees, with a qR pattern observed in lead aVL. Additionally, there was evidence of left atrial overload.
