A onda P tem duração de ≥ 120 ms em DII, associada à onda P com porção negativa ≥ 40ms em V1, E/OU onda P com duplo pico separado por mais de 40 ms ou padrão plus-minus em DII, E/OU Índice de Morris positivo: porção negativa da onda p em V1 ≥ 40ms de duração e amplitude negativa ≥ 0,1mV. 

 The P wave lasts ≥ 120 ms in DII, associated with a negative portion of the P wave ≥ 40 ms in V1, AND/OR P wave with double peak separated by more than 40 ms or plus-minus pattern in DII, AND/OR Positive Morris index: negative portion of the p wave in V1 ≥ 40ms in duration and negative amplitude ≥ 0,1mV.

 Paciente feminina, 45 anos, com queixa de palpitações. Ao ECG, apresenta ritmo sinusal regular com onda P de 144 ms de duração ( ≥ 120 ms), com duplo pico separados por mais que 0,04 s em DII e com área da porção negativa da onda P em V1 maior ou igual a 40 mm/s, correspondendo a sobrecarga atrial esquerda . Observa-se, também, alterações inespecíficas da repolarização ventricular.  

 45-year-old female patient presenting with palpitations. On ECG, tracing shows regular sinus rhythm with P wave duration of 144 ms ( ≥ 120 ms) , with double peak separated by more than 0.04 s in lead DII and negative terminal force in lead V1 ≥40 mm², consistent with left atrial enlargement . Nonspecific ventricular repolarization abnormalities are also noted.

 Paciente masculino, 65 anos, com queixa de dor torácica. Ao ECG, apresenta ritmo sinusal com onda P de 152 ms ( ≥ 120 ms), com duplo pico separado por mais que 0,04 s em DII e com área da porção negativa da onda P em V1 maior ou igual a 40 mm/s, compatível com Sobrecarga Atrial Esquerda. Observa-se também extrassistolia ventricular monomórfica e isolada.

On ECG, tracing shows sinus rhythm with P wave duration of 152 ms ( ≥ 120 ms), with double peaks separated by more than 0.04 seconds in lead II, and the negative portion of the P wave in lead V1 has a larger area, equal to or exceeding 40 mm/s, consistent with left atrial enlargement . Additionally, monomorphic and isolated ventricular extrasystoles are observed.
