Paciente de 32 anos, sexo masculino, em ECG de rotina. Ao ECG, apresenta ritmo sinusal regular, com complexo QRS tem duração de 110 ms, normal, eixo elétrico em -143 graus (desvio extremo), com progressão reversa da onda R de V1-V6 e baixa voltagem de V3-V6. A repolarização ventricular processou-se com inversão de onda T em DI. Os achados são compatíveis com **dextrocardia**, sendo recomendado excluir inversão de eletrodos.

A 32-year-old male patient underwent routine ECG. The ECG showed regular sinus rhythm, with a normal QRS complex lasting 110 ms, electrical axis at -143 degrees (extreme axis deviation), with reverse progression of the R wave from V1-V6 and low voltage from V3-V6. Ventricular repolarization occurred with T wave inversion in DI. These findings are consistent with **dextrocardia**, and it is recommended to exclude electrode inversion.
