Paciente masculino, 31 anos, exame realizado para acompanhamento clínico. Apresenta HAS e história familiar positiva de doença coronariana. Ao ECG, apresenta QRS alargado, com padrão rsR´ em V1 e ondas S empastadas em V5 e V6. Eixo mais negativo que -45º, desviado à esquerda, d com padrão qR em aVL, compatíveis com B**loqueio de Ramo Direito e Bloqueio Divisional Anterossuperior do ramo esquerdo.** Observa-se, também, sobrecarga atrial esquerda.

*31-year-old male patient undergoing clinical follow-up examination. He has systemic arterial hypertension and positive family history of coronary artery disease. On ECG, he presents widened QRS with rsR' pattern in lead V1 and slurred S waves in leads V5 and V6. Axis more negative than -45°, showing left axis deviation, with qR pattern in lead aVL, consistent with **right bundle branch block and left anterior fascicular block.** Left atrial enlargement is also observed.*
