*A 42-year-old female patient presenting with tachycardia. She has been diagnosed with hypertension and has previos history of myocardial infarction. She is currently taking ACE inhibitors. The ECG shows a regular sinus rhythm with a heart rate of 68 bpm. The P wave lasts 116 ms, and the area of the negative portion of the P wave in lead V1 is greater than or equal to 40 mm/s. The QRS complex has a normal morphology and lasts 80 ms. Pathological Q waves are observed in leads V1, V2, and V3, corresponding to **electrically inactive area** in the anteroseptal wall, potentially associated with a previous myocardial infarction. Left atrial enlargement and rightward deviation of the QRS axis can also be observed.*

- Exemplo 2 | Example #2

*A 48-year-old male patient undergoing an elective examination. He has been diagnosed with hypertension and is currently taking clopidogrel and losartan. On the ECG, tracing shows regular sinus rhythm with a heart rate of 56 bpm. The QRS complex appears normal in morphology, with a duration of 106 ms, and the electrical axis is deviated to the left. Additionally, a pathological Q wave is observed in leads II, III, and aVF, corresponding to **electrically inactive area**, suggesting a possible previous myocardial infarction. Ventricular repolarization shows a corrected QT interval of 457 ms, which is prolonged. There are primary alterations in ventricular repolarization noted in leads II, III, aVF, V4, V5, and V6.*

Paciente do sexo masculino, 41 anos, realiza o exame devido a angioplastia prévia.

*A 41-year-old male patient is undergoing an examination following previous percutaneous coronary intervention. His medical history includes hypertension, prior myocardial infarction, obesity, smoking, and a family history of coronary artery disease. Currently taking aspirin, losartan, metoprolol succinate, rosuvastatin, and escitalopram. The ECG shows a regular sinus rhythm with a heart rate of 77 bpm. A pathological Q wave is noted in leads V1, V2, V3, and V4. Additionally, negative, symmetrical, and pointed T waves are observed in leads V2, V3, V4, and V5. These findings indicate subepicardial ischemia and suggest the presence of an **electrically inactive area** in the anteroseptal wall, which raises concerns about potential coronary artery disease and a prior myocardial infarction.*

- Exemplo 4 | Example #4

Paciente do sexo masculino, 63 anos, realiza o exame para avaliação pré-anestésica. Hipertenso.

*A 63-year-old male patient undergoing pre-anesthetic examination. He has a history of hypertension. The ECG shows a regular sinus rhythm with a heart rate of 58 bpm. The QRS complex is enlarged, with a normal morphology lasting 118 ms and a normal electrical axis. Pathological Q waves are observed in leads V1, V2, V3, and V4. Additionally, negative, symmetrical, and pointed T waves with an amplitude between 1 and 5 mm are noted in leads V1, V2, V3, and V4. There is no evidence of bundle branch block, left ventricular hypertrophy, pacemaker presence, or ventricular pre-excitation. These findings indicate primary changes in ventricular repolarization and suggest an **electrically inactive area** in the anteroseptal wall, possibly due to a previous myocardial infarction.*
